In discussions about the health benefits of coffee drinking, the effects of coffee are often confused with those of caffeine. While it is true that many of the effects of coffee are caused by caffeine, coffee also has several interesting and beneficial non-caffeine related effects.
Chlorogenic acid in Green Coffee Bean - a key player
Coffee contains a myriad of biologically active substances, and recent research has identified chlorogenic acid as being one of them. Chlorogenic acid is present in highest amounts in the coffee bean and the seeds inside the coffee berry. Green (or raw) coffee is a major source of chlorogenic acid. Commercial preparation of coffee destroys a lot of the chlorogenic acid and other beneficial polyphenolic substances, but heavy coffee drinkers may still ingest from 0.5 to 1.0 g chlorogenic acid per day. Anti-obesity effects of Green Coffee Bean Extract
Several studies in both humans and animals have showed that green coffee bean extract holds great potential as an anti-obesity agent. A study investigating the effect of green coffee bean extract on fat accumulation and body weight in mice demonstrated weight loss with a reduced abdominal (visceral) and hepatic fat content. This effect was attributed to activation of fat metabolism in the liver. Another study in obese mice fed a high-fat diet found that chlorogenic acid not only significantly lowered body weight, visceral fat mass and insulin levels, but also reduced triglyceride levels and cholesterol levels. In addition, it was found that chlorogenic acid also elevated adiponectin levels in the blood (more on this below).
In a study on hamsters fed a high-fat it was found that chlorogenic acid beneficially affected both glucose and fat metabolism, as indicated by a significant reduction in levels of fasting blood triglycerides, free fatty acids, total cholesterol, low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), glucose (FSG), and insulin. This study also found a reduction in live fat content.
In humans, supplementation of 200 mg green coffee extract per day results in an average weight loss of (2.5 kg) in 12 weeks. Higher daily doses up to 1050 mg (350 mg three times daily) of the standardized green coffee bean extract (Svetol®) have induced a larger magnitude a weight loss of 8 kg in overweight individuals after 22 weeks, who also adhered to diet and exercise advice. The higher dose was well tolerated and did not cause any adverse effects.
How does it work?
In contrast to caffeine, which is a stimulant that increases caloric expenditure and stimulates fat burning and thereby weight loss, green coffee bean induces fat loss via non-stimulant mechanisms. First, green coffee bean slows down the absorption of glucose from the small intestine and lowers after-meal blood glucose levels; the importance of this can be illustrated with the well documented effects seen with low glycemic index foods (enhanced fat burning, increased satiety and fat loss), which also slow down the absorption of glucose and lower blood glucose levels.
Second, green coffee bean inhibits carbohydrate digesting enzymes and the actual absorption process itself. This means that less glucose gets taken up by the body, and thereby less calories.
Third, green coffee bean also shifts the site of glucose absorption to more distal parts of the intestine, which in turn enhances the secretion of the gut hormone GLP-1 (glucagon-like peptide 1). This is an interesting effect in that GLP-1 currently is generating much scientific attention due to its involvement in satiety control, glucose homeostasis, and the cardiovascular system. The pharmaceutical industry is developing anti-diabetic drugs that act on GLP-1 to cause weight loss in overweight and obese patients, reduce blood pressure and improve endothelial function and vasodilatory effects. The GLP-1 weight loss effect has been attributed to a reduction in food intake and slowing of gastric emptying.
Fourth, chlorogenic acid in green coffee bean significantly inhibits fat synthesizing enzymes, while increasing the activity of fat burning enzymes. Finally, as mentioned above, green coffee bean also increases blood levels of adiponectin, a hormone that is secreted from fat cells. Adiponectin enhances fat burning and also increases insulin sensitivity, lowers blood glucose (blood sugar) and reduces triglyceride levels .
Anti-diabetic - suppression of after-meal blood sugar spikes
Another major effect of green coffee bean is its ability to significantly lower fasting blood glucose levels, and especially dampen after-meal blood glucose surges.
Why is this important and how many of us are affected?
35% of Americans aged 20 and older, and half of those aged 65 and older have pre-diabetes. The prevalence of undiagnosed diabetes is 27%. The high prevalence of pre-diabetes and undiagnosed diabetes is alarming since elevated fasting blood glucose levels even within the "normal" range greatly increase the risk for development of full-blown type 2 diabetes, stomach cancer, and heart attack. Fasting blood glucose values in the upper normal range are an important independent predictor of cardiovascular death even in non-diabetic apparently healthy people.
A mounting body of evidence shows that high after-meal blood glucose spikes inflict as much or even more damage than high fasting blood sugar. Among individuals with "normal" blood glucose readings, heart attack risk increases by 58% for a 21 mg/dl increase in after-meal blood sugar level 45, and 2 hour after-meal blood glucose levels are a strong determinant of atherosclerosis (diagnosed by IMT, intima-media-thickness). It is well established that after-meal blood glucose spikes are an independent risk factor for heart attack and all-cause mortality. More specifically, people with the highest after-meal glucose levels, (range 150-194 mg/dL) have a 27% greater risk for cardiovascular disease compared with those with the lowest levels, (range 69-107 mg/dL.)
How effective is green coffee bean extract?
In mice, green coffee bean extract was shown to dampen the elevation of blood glucose by 43% after a heavy meal. A notable study in humans tested different dosages of a standardized green coffee bean extract containing 50% chlorogenic acid. The subjects were given doses from 100 mg up to 400 mg and then ingested 100 g glucose (which is a standard method of testing of after-meal blood sugar elevations). It was found that after-meal blood glucose levels dropped by an increasingly greater amount as the dosage of the green coffee bean extract was raised. In the group of human subjects who took the 400 mg dose, there was a 24% drop in just 30 minutes after glucose ingestion, and a 32% drop after 2 hours. This means that if you had a dangerous after-meal glucose reading of 160 mg/dL, green coffee bean extract supplementation could slash it to 109 mg/dL, and thereby also markedly lower your health risk. It is not surprising that the anti-diabetic effect of coffee has in large part been attributed to chlorogenic acid, and that it counteracts the negative effect of caffeine on glucose metabolism insulin sensitivity.
What determines your blood sugar levels?
While the amount of carbohydrates that you eat, coupled with how well your pancreas are working, have a significant effect on your blood sugar levels, this isn't the whole picture. There is an enzyme in the liver called glucose-6-phosphatase (G6P), which is a big player in the blood glucose equation. G6P releases glucose from stored glycogen in the liver into the bloodstream, and also converts protein to glucose. Among physically inactive people and those who tend to store body fat around the waist, the activity of G6P is elevated, which can throw your blood glucose levels off the chart. Stress also greatly ramps up G6P, and can wreak havoc with your blood glucose regulation.
It is here that chlorogenic acid comes to rescue; apart from inhibiting and slowing down the absorption of glucose, chlorogenic acid also inhibits the activity of G6P, which will help keep your blood glucose levels on track. According to the guidelines, your after-meal blood glucose should never exceed 140 mg/dL. If you want to test yourself, ask your doctor to give you a diabetes kit. Ideally, you want your after-meal blood level to be below 110 mg/dL. If it is higher than that, green coffee bean extract supplementation should be a supplement in your dietary regimen.
