The Health Benefits of Policosanol
The normal diet in general will have lipids along with other nutritional factors. These lipids are absorbed and transported in the blood for various uses including, reserve energy. Several factors during aging give rise to increased circulation of lipids in the blood. This leads to several lipid-associated disorders. Lipids are insoluble in water. Due to this reason, the lipids are transported in the blood by the formation of lipid-phospholipid-protein complex (lipoproteins), which are hydrophilic. The protein part (called apoprotein) act as binding sites and enzyme cofactors, involves in the metabolism of the various particles. Based on the ratio of the constituents of lipoproteins and their size they are classified into VLDL, LDL and HDL. Each of them have different functions in the body. The LDL are involved in the carrying of cholesterol from the liver to the peripheral tissues and HDL back to the Liver and prevents the lipid accumulation in arterial walls and is inversely related to the incidence of coronary artery disease. Hence HDL is called good cholesterol and LDL is called bad cholesterol. Several pharmacological interventions are somewhat successful in controlling LDL in blood but have side effects. - Bee's wax: It is made up of monoesters, hydrocarbons, free fatty acids and free alcohols. Bee's wax contains mixture of higher molecular weight and primary aliphatic alcohols that contain 24 - 34 carbon atoms.
- Sugarcane wax: It contains a mixture of higher primary aliphatic alcohols of 22 to 30 carbon atoms. Sugarcane wax contains higher percentage of 1-Octacosanol 60-70% as compared to bee wax 30-60% 1-Triacontanol, 1-Dotriacontanol and Tetracosanol percentage are higher in bee wax as compared to sugarcane wax.

In normocholesterolemic volunteers without dietary restrictions, 10 or 20 mg of Policosanol per day given for 4 weeks decreased total and LDL cholesterol in a dose dependent manner27. Treatment with 5 or 10 mg per day over a 6 or 8 week period in type II hypercholesterolemia patients lowered total cholesterol by about 13 % or 16 % and LDL-C by about 18 % or 22 %, respectively28, 29. In a separate short-term studies over 6 to 8 weeks that used incremental dose upto 20 mg per day reduced cholesterol in a dose dependent and that, with, LDL-C reduced by about 30 %23,30. In addition, the ratios of total and LDL cholesterol to high-density lipoprotein cholesterol (HDL-C) can be substantially lowered in the order of magnitude of 30 %. Policosanol also increased HDL-C in type II hypercholesterolemia28-32. Accordingly, the ratios of total or LDL cholesterol to HDL-C, which are considered important parameters to characterize beneficial effects on lipid profiles in terms of cardiovascular risk, were improved substantially. In this respect, Policosanol seems to be superior to both statins and fibrates.Effect on Diabetic Hypercholesterolemia:
At a dosage of 10 mg/day, it was found to significantly reduce total cholesterol by 17.5% and LDL-C by 21.8% from both baseline and placebo-control values in patients with non-insulin-dependent diabetes (NIDDM), potentially decreasing their risk for coronary artery disease3.Triglyceride concentrations, glucose levels, and hemoglobin (Hb) A1 values remained unchanged. Policosanol is as effective as Aspirin on Platelet Aggregation:
A randomized, double blind, placebo-controlled study was conducted in 43 healthy volunteers to compare the effects of Policosanol (20 mg/day), aspirin (ASA) (100 mg/day) and combination therapy (Policosanol 20 mg/day plus ASA 100 mg/day) on platelet aggregation. The healthy volunteers were randomly treated for 7 days. Both, platelet aggregation and coagulation time were measured at baseline and after therapy. When Policosanol was administered platelet aggregation induced by ADP (37.3%), epinephrine (32.6%) and collagen (40.5%) were significantly reduced. Meanwhile, aspirin significantly reduced platelet aggregation induced by collagen (61.4%) and epinephrine (21.9%) but not ADP-induced aggregation. Combined therapy significantly inhibited highest reductions of platelet aggregation induced by collagen (71.3%) and epinephrine (57.5%). Aggregation induced all the agonists reaching the Coagulation time did not change significantly in any group. Four volunteers reported mild adverse experiences during the study: three ASA-treated cases referred headache, epigastralgia and nose bleeding, meanwhile one patient receiving combination therapy reported gum bleeding. The present results demonstrate that Policosanol (20 mg/day) is as effective as ASA (100 mg/day). Policosanol reduces platelet aggregation by altering prostaglandin synthesis. Specifically, Policosanol lowers serum levels of the pro-aggregatory thromboxane A2, while increasing the anti-aggregatory prostaglandin prostacyclin. Clinical trials in humans have shown that Policosanol significantly inhibits platelet aggregation comparable to low dose aspirin. Policosanol reduces Blood Pressure:
Policosanol significantly reduced LDL-C (-19.1 %), total cholesterol (-13 %) and the ratios of cholesterol to HDL-C (-20 %) and LDL-C to HDL-C (-24.2 %) in hypertensive patients with hypercholesterolemia, while significantly increasing HDL-C levels (+17.1 %) 22. After 12 months of therapy Policosanol significantly lowered systolic pressure (-10 mm Hg), while in the placebo group the values remained unchanged. Many of the patients were on beta-blockers and diuretics, two classes of drugs known to adversely impact blood lipid levels.
