Iodine
Iodine is a trace mineral needed to make thyroid hormones, which are necessary for maintaining normal metabolism in all cells of the body.

Where is it found?
Seafood, iodized salt, and sea vegetables—for example, kelp—are high in iodine. Processed food may contain added iodized salt. Iodine is frequently found in dairy products. Vegetables grown in iodine-rich soil also contain this mineral.

Who is likely to be deficient?
People who avoid dairy, seafood, processed food, and iodized salt can become deficient. Iodine deficiency can cause low thyroid function, goiter, and cretinism. Although iodine deficiencies are now uncommon in Western societies, the U.S. population has shown a trend of significantly decreasing iodine intake from 1988–1994. If this trend continues, iodine deficiency diseases may become more common.

How much is usually taken?
Since the introduction of iodized salt, iodine supplements are unnecessary and not recommended for most people. For strict vegetarians who avoid salt and sea vegetables, 150 mcg per day is commonly supplemented. This amount is adequate to prevent a deficiency and higher amounts are not necessary.

Are there any side effects or interactions?
High amounts (several milligrams per day) of iodine can interfere with normal thyroid function and should not be taken without consulting a doctor. Although potassium iodide supplementation (prescribed for some skin disorders) is usually well-tolerated, it has been known to produce adverse reactions such as rashes, itching or lesions on the skin, gastrointestinal symptoms, or hypothyroidism, especially in people with a prior history of thyroid problems. Because of such potential problems, the use of potassium iodide therapy should be supervised by a doctor. The average diet provides about four times the recommended amount of iodine. For susceptible people, that amount of iodine may be enough to cause health problems. A possible link to thyroid cancer has been observed in areas where an iodine-rich diet is consumed, and among populations that supplement with iodine. However, there is insufficient evidence to conclude that iodine supplementation is responsible for the increased incidence of thyroid cancer. Some people react to supplemental iodine, the first symptom of which is usually an acne-like rash.

When people with small, nontoxic goiter (living in areas not deficient in iodine) received iodine injections, they had a higher incidence of abnormal antibodies suggestive of the early stages of autoimmune thyroid disease.

Children with iodine deficiency may also have iron-deficiency anemia, and this anemia may interfere with the therapeutic action of iodine supplementation. Correcting iron deficiency in such children with iron supplements has been shown to improve the efficacy of oral iodine in treating goiter.

Kelp
Kelp is a sea vegetable that is a concentrated source of minerals, including iodine, potassium, magnesium, calcium, and iron. Kelp as a source of iodine assists in making thyroid hormones, which are necessary for maintaining normal metabolism in all cells of the body.

Where is it found?
Kelp can be one of several brown-colored seaweed species called Laminaria.

Who is likely to be deficient?
People who avoid sea vegetables, as well as dairy, seafood, processed food, and the salt shaker, can become deficient in iodine. Iodine deficiency can cause low thyroid function, goiter, and cretinism. Although iodine deficiency is now uncommon in Western societies, the U.S. population has shown a trend of significantly decreasing iodine intake.

Since the introduction of iodized salt, additional sources of iodine, such as kelp, are not necessary for most people. However, kelp can be consumed as a source of other minerals. A report from Great Britain indicated that the average kelp-based supplement contained 1,000 mcg of iodine (the adult RDA in the United States is 150 mcg per day). It has been suggested that intakes above 2,000 mcg per day should be regarded as excessive or potentially harmful.

Are there any side effects or interactions?
There have been several case reports of high intakes of kelp providing too much iodine and interfering with normal thyroid function. People with thyroid disease should check with a doctor before taking supplements that contain kelp.

Selenium
Selenium is an essential trace mineral.

Where is it found?
Brazil nuts are the best source of selenium. Yeast, whole grains, and seafood are also good sources. Animal studies have found that selenium from yeast is better absorbed than selenium in the form of selenite.

Who is likely to be deficient?
While most people probably don’t take in enough selenium, gross deficiencies are rare in Western countries. Soils in some areas are selenium-deficient and people who eat foods grown primarily on selenium-poor soils are at risk for deficiency. People with AIDS have been reported to be depleted in selenium. Similarly, limited research has reported an association between heart disease and depleted levels of selenium. People who are deficient in selenium have an increased risk of developing certain types of rheumatoid arthritis.

How much is usually taken?
While the Recommended Dietary Allowance for most adults is 55 mcg per day, an adult intake of 100–200 mcg of selenium per day is recommended by many doctors.

Are there any side effects or interactions?
Selenium is safe at the level people typically supplement (100–200 mcg); however, taking more than 900 mcg of selenium per day has been reported to cause adverse effects in some people. Selenium toxicity can result in loss of fingernails, skin rash, and changes in the nervous system. In the presence of iodine-deficiency-induced goiter, selenium supplementation has been reported to exacerbate low thyroid function. Although most research suggests that selenium prevents cancer, one study found an increased risk of a type of skin cancer (squamous cell carcinoma) in people taking selenium supplements. The National Academy of Sciences recommends that selenium intake not exceed 400 mcg per day, unless the higher intake is monitored by a healthcare professional. In a double-blind study of people who took 200 mcg of selenium per day for several years to prevent recurrences of skin cancer, the incidence of diabetes was higher in people who received selenium (9.7%) than in those who received a placebo (6.5%). While this difference was statistically significant, this finding should be considered preliminary, since the study was not originally designed to test whether selenium influences the risk of developing diabetes.
